Specifications include, but are not limited to: HHSC PCS is seeking competitive bids to establish contract(s) for a Certified Orientation and Mobility Specialist for the residents at Abilene State Supported Living Center (AbSSLC). Abilene State Supported Living Center is a state operated facility assisting both male and females who are diagnosed with intellectual disabilities, autism, and a variety of psychological diagnoses and behavioral issues. AbSSLC residents are provided care in the least restrictive environment and taught independence skills for daily life. A Certified Orientation and Mobility Specialist (COMS) works in conjunction with the Interdisciplinary Team (IDT) to support/improve functional mobility and independence of residents with impaired sight. 1. COMS shall present and submit a treatment plan proposal to the InterdisciplinaryTeam (IDT) based on each resident’s needs and proposed goals upon completion of the resident’s assessment. 2. COMS sessions shall be conducted on‐site. 3. COMS shall visit the scheduled resident(s) monthly. 4. The COMS shall provide to the AbSSLC Habilitation Therapy Director a schedule that must include days, times and names of residents to be seen. The COMS shall make every effort to adhere to the schedule. If the schedule must be adjusted, the COMS shall contact the AbSSLC Habilitation Director immediately. 5. COMS shall maintain a record of residents receiving services. The record shall include the individual receiving the session, date of session, and start and finish time. The COMS shall document the session in Electronic Health Record (HER) State system. 6. COMS shall be familiar with Positive Behavior Support Plan (PBSP) for each resident. The COMS shall attend and participate in all meetings as requested by AbSSLC. 7. COMS shall complete monthly progress notes to include areas addressed in treatment sessions and document the progress or regression from month to month.
