Specifications include, but are not limited to: The required services to be provided are as-needed, statewide Biomedical Destruction services (by way of incineration and/or landfill according to applicable local, state, and federal laws) for State owned goods (such as supplies, equipment, pharmaceutical) for the Department of State Health Services (DSHS). a) Contractor shall recycle items to recover materials that can be repurposed or reused such as metals, plastics, and papers to reduce the burden on landfills and environmental pollution. Equipment and supplies may not be salvaged or reconditioned in whole for sale or redistribution. b) Contractor shall be responsible for destruction of liquid/solid mixture materials and solid without free liquid materials from DSHS contracted storage locations (Section 7.4). Awarded Contractor shall furnish all labor, tools, transportation, equipment, materials and supplies as required to complete the service request. c) Awarded Contractor shall be responsible for supervision, clean up and disposal, and must have all applicable licenses, permits and insurance coverage. Services shall include removal, transport, and delivery to approved waste facilities in accordance with Texas Commission on Environmental Quality (TCEQ) and the Environmental Protection Agency (EPA) regulations. d) Awarded Contractor shall respond to the Contract Administrator within twenty-four (24) hours after the initial request for an estimate. e) Contractor shall invoice all materials at an amount not greater than the cost of materials plus 10% mark up. f) Contractor shall check in and out with the authorized DSHS representative to ensure the representative annotates the start and completion times on the service ticket for the services performed. Contractors shall provide the following information on the service ticket: project name, name of personnel performing the work, trade category of person performing work, number of personnel, and hours worked. Contractor invoice must match approved service tickets. Contactor material invoices shall be attached to final invoice. Travel time to and from the job site is not reimbursable under this Contract. g) Contractor shall be responsible for examination of all facilities where the work will take place and to be familiar with the conditions under which the work will be completed.
