Specifications include, but are not limited to: The required services to be provided are for as needed rentals of CPAP and BiPAP machines and applicable accessories for clients at Rusk State Hospital (RSH), which houses approximately 288 patients. Contractor shall have on hand and available replacements for all rented equipment at no additional expense to RSH. Contractor shall have additional equipment available for new prescriptions as needed. Equipment may be added or removed during the term of the awarded Contract at the sole discretion of RSH. Exhibit H Equipment Pricing List is not all-inclusive nor a guarantee of rental services. RSH may request specialized CPAP or BiPAP equipment per prescription, including pricing and availability, as applicable. All CPAP and BiPAP machines shall include, at minimum: a) Facemask (per patient prescription) b) Headgear c) Filters d) Tubing; Contractor shall include all materials, quantities, devices, equipment, and labor including repairs due to normal wear and tear, to provide a working system, complete in all aspects for patient use per each rental. a) Contractor shall supply, at no additional cost to RSH, all parts and supplies that require replacement from normal wear and tear. Contractor shall ensure all equipment consumable parts and materials shall be new and supported by the manufacturer. Contractor shall ensure all rental equipment shall be cleaned and sanitized prior to shipment to RSH. Contractor shall warrant the machine is suitable for normal operation and use at the time of delivery and shall replace defective equipment at no charge to the facility. Contractor shall provide manufacturer and service warranty details of all machines and equipment upon request by RSH. After receiving order from RSH, all parts and supplies shall be shipped and received on-site within five (5) business days. All shipping costs shall be included submitted bid pricing.
