Assessment and Evaluation: 1) Typed evaluations will be provided to the Community Corrections Officer within 7 days of initial assessment of each offender referred for services that will include a recommended course of treatment. 2) Service Agency must contact DEPARTMENT within twenty-four (24) hours whenever any Defendant fails to comply with his or her recommended treatment, including failure to show for initial appointment or unauthorized departures. Treatment Services: 1) Coordinate with DEPARTMENT to identify needs of Defendants that are beyond the scope of CONTRACTOR’S Services and make appropriate referrals in such circumstances. 2) Contact DEPARTMENT within twenty-four (24) hours whenever any Defendant fails to comply with his or her recommended treatment, including failure to show for initial appointment or unauthorized departures. 3) Document on a weekly basis of the Defendant’s level of participation and compliance with treatment goals and objectives. 4) Immediately notify (24 hours) of high-risk behavior and/or technical violations of probation in written format to the Community Corrections Officer. 5) Provide monthly typed progress reports to the Community Corrections Officer for each offender by the 10th calendar day of the month. The progress report will address the offender’s progress, or lack thereof, toward treatment goals and specific issues identified in the offender’s treatment plan. 6) The clinician must be available to testify, without cost, as a witness in hearings involving offenders assessed or treated in their program and the clinician must be available to discuss assessment and treatment recommendations with the CSCD. 7) Treatment groups will be limited to a maximum of eight people...
