Key responsibilities include: 1. Onboarding and MSSP Participation Preparation o Guide the practice through the MSSP enrollment process for 2026 o Facilitate the development of required reporting and contracting documentation o Assess and recommend TIN segmentation or alignment strategies to optimize attribution and cost performance 2. Data Integration and Analytics o Provide access to a secure data platform for population health reporting, quality tracking, and claims analytics o Deliver regular, actionable performance reports to practice leadership and providers o Support integration with EHR systems and workflows where possible 3. Care Coordination and Management o Offer tools, resources, and guidance for risk stratification, patient outreach, and care gap closure o Collaborate with practice leadership to support care coordination personnel, either through direct staffing or shared resources o Align care models to reduce unnecessary utilization and improve chronic care outcomes
