Specifications include, but are not limited to: The Youth Assessment Center seeks to reduce juvenile crime and serve as a central hub for network coordination through the provision of appropriate services to justice involved youth ages 12 - 17 and their families. Evaluative services will include but not limited to; Behavioral Health (mental health, substance use, trauma, etc.) Human Trafficking Coordination of Release (home/community, juvenile detention, department of human services) Voluntary follow-up (contact with families to ensure connection to services) Individualized Assessments (triages to diffuse crisis and connect to community partners) Individualized plans (referrals to community partners and resources base on need) Case management to assist families navigate resources Follow-up contact with families to ensure connection to services Community education
