The Contractor shall be duly registered and licensed as a pharmacy in the State of Tennessee. Evidence of current licensing will be kept on file at the State’s home office and at each Facility operated by the State. The Contractor shall also provide evidence of current licensure or certification in the State of Tennessee of all pharmacists and pharmacy technicians providing services under this Contract. A.4. The Contractor shall comply with all applicable laws, rules, and regulations. A.5. The Contractor shall provide, furnish, and supply Pharmaceutical Goods, supplies, and equipment to each Facility during the term of this contract: A.6. The Contractor shall establish a formulary with the input and written approval of TSVHB administrative staff. A.7. The Contractor shall provide a twenty-four (24) hour per day, seven (7) day per week computerized unit dose drug delivery system, and all compounding of same, in such types and quantities as each Facility may request from time to time. The Contractor shall ensure that medications are dispensed in their generic form unless the physician writes “dispense as written” on the prescription. The Contractor shall not provide over-the-counter medications.
