Vendor will provide non-emergency medical transportation for clients and/or patients with or without medical needs and/or conditions to and from medical, pharmacy, dental and behavioral/mental health, other ancillary/specialty healthcare services or designated sites providing mobile health services appointments within 1-3 hours of notification Monday through Friday between the hours of 5AM – 6PM with the option for transportation on Saturdays 5AM – 6PM. Counties that will need be covered for transportation include but are not limited to: Davidson, Cannon, Cheatham, Dickson, Hickman, Macon, Robertson, Rutherford, Smith, Sumner, Trousdale, Williamson, and Wilson. Vendor must be able to accommodate families with up to 7 individuals, patients who require medical assistance such as oxygen, wheelchair, motorized wheelchair, chair lift, have illnesses, special needs, car seat requirements, or behavioral or mental limitations. Vendor must be able to accommodate patient(s) and families with multiple appointments in a day. Vendor must carry all applicable state licensing requirements, certifications, and insurance(s) required by law for the business and all drivers. All Drivers must have valid Department of Transportation (DOT) background checks, maintain an active driver's license for the duration of the contract, and have a current DOT Physical on file in compliance with DOT health and safety standards. Firm will ensure that all drivers are substance/drug free before transporting clients. Firm must be able to provide proof of random alcohol and or drug testing for all driver(s) assigned to transport clients on behalf of the Metro Public Health Department. Vendor must have a minimum of 3 years' experience providing transportation of similar scope and service to an entity as large as metro. All driver's transporting clients and patients on behalf of Metro must carry an active driver's license and have a completed criminal background check on file. The firm must provide this documentation upon request throughout the duration of the contract.
