Bidder shall provide all necessary facilities, materials, supplies, equipment, supervision, and personnel and other items and services to perform the lead-based paint mitigation via encapsulation services. The scope of work for this RFQ includes but is not limited to: Arrival and Communication - Provide minimum of 48-hour notice before working inside occupied apartments. - Notify the MDHA Maintenance Supervisor upon arrival and departure from work site - Provide MDHA Maintenance Supervisor with a designated Single Point of Contact (SPOC) name and phone number. The SPOC shall be on call at all times while work is being performed. Work Description - Evaluate the surfaces requiring mitigation to select the most appropriate materials to successfully mitigate the hazard. - Comply with manufacturer instructions for all materials used. - Provide documentation of mitigation including existing condition and mitigation completion photos; activity log verifying date/specific location/brief description of services; certified supervisor name of mitigation activities; signature of certified supervisor. Compliance and Safety - Use personal protective equipment (PPE): respirators, gloves, disposable coveralls. - Comply with HUD Guidelines for the Evaluation and Control of Lead-Based Paint Hazards and ensure protection for workers, residents and members of the public. - Comply with local and state codes and regulations. - Comply with MDHA's Lead Hazard Control Plan specifies mitigation, via "encapsulation", as an interim control measure to reduce the risk of exposure to lead paint. - Post warning signs, restrict access, establish containment, and shut down HVAC systems
