1.Provide a synopsis of your firm including when and where incorporated, major business activities, strategic planning. Describe the company structure (private, public, partnership, etc.) The Contractor shall provide services for claims administration for Shelby County Government’s On the Job Injury program. Services shall include, but not be limited to: 3. Auditing reports of employee injuries in order to determine compensability 4. Contacting employees for recorded statements, 5. Follow-up call to department management to verify information provided on the OJI Incident Report. 6 Auditing and paying eligible bills for medical care, 7 Providing cost containment services, 8. Providing medical case management services, where indicated and approved by Shelby County Risk Management. 9. Providing computer generated claim reports, 10. Establishing and recording appropriate medical and indemnity reserves, 11. Providing on-line access to claims and information, 12. Providing vocational rehabilitation services, 13. OSHA record keeping & reporting, 14. Develop and issue 1099 forms on behalf of the County, and compliant to federal and state regulations 15. Develop and provide as requested by Shelby County Government an annual SOC 1 report or SSAE, report compliant to Accounting Standards Board requirements. 16.
