3859 - Miscellaneous Forms and Envelopes

Knoxville Utilities Board (KUB) is searching for a vendor to supply miscellaneous forms and envelopes for our print shop. KUB will only be awarding to 1 (one) vendor and the winning vendor must be able to manufacture all items in-house each time we order them for the duration of the contract. The one exception we will consider is the Kraft Envelopes with metal clasp. Those could potentially be outsourced to a third-party.

Please read the RFP in its entirety before submitting a proposal, and enter pricing in the attached Excel spreadsheet labeled "Attachment E".
Include all required documentation listed in the RFP with your proposal, and make note of all other attachments.
KUB would like to enter into a supply agreement with the winning vendor. The term for the supply agreement will be 3 years initially with the option for two additional one-year renewals.
