Specifications include, but are not limited to: A.1. Provider's health service staff shall work cooperatively with SD Mines student development staff to provide proactive intervention in the form of health and wellness education. 2. The Health Service Office staffing and hours will be established for the year by mutual agreement and must include physician/P.A./L.N.P. care on site. The office must be open during the fall and spring semesters (when class is in session). SD Mines is asking for proposals that provide three different pricing options for the hours open per week: 25 hours, 30 hours, and 35 hours. Provider also agrees to attend New Student Orientation sessions. 3. Provider shall deliver routine and acute outpatient medical care of all covered students including the first day of each semester to the final day of the semester. Covered students are enrolled university students who have paid the Student Health Service fee to SD Mines for the semester that medical services are provided. Services shall include acute care of illness or injury and other care as described below. 4. Covered students enrolled in summer sessions shall receive medical services identical to those provided to covered students at the Student Health Office during the academic year except that such medical services may be provided at the Provider's clinic. 5. For care after hours, on holidays, or on weekends, Provider shall specify options for covered students and shall offer a discounted rate. 6. Provider further agrees to participate in the planning and implementation of the annual Health and Wellness Expo. Provider shall participate in educational sessions as needed or requested by SD Mines. 7. Provider agrees to work with a Student Health Service Advisory Committee on educational topics and any other student health-related concerns. 8. Services shall be delivered by Provider employees only and may not be subcontracted without prior written approval by SD Mines. 9. Provider shall provide limited on-site laboratory services for the diagnosis and treatment of acute illnesses, including dip urinalyses and group A strep screens. 10. Provider shall provide the following STI testing limited to Chlamydia, Gonorrhea, Trichomoniasis, HSV, and HIV. SD Mines is asking for proposals that provide pricing options for on-campus and off-campus testing of the above listed items.
