Possess ownership of a cloud-based, secure, and HIPAA-protected SaaS technology platform and products that can ingest EHR patient files from multiple healthcare sites, identify tobacco users from these EHR files, transfer those ready to quit through a seamless electronic process to the SC Tobacco Quitline, and receive outcome reporting back from the Quitline on all referral files. Maintain current services for SC providers who have electronic referring to the Quitline implemented through the eHub. Provide a closed-loop bidirectional file transfer service from SC provider EHR systems to the Quitline, basically converting them from using a fax referral system to an electronic referral system. Provide current technology systems required to conduct SFTP or higher file transfers of patient referrals from a variety of SC health systems, hospitals, and individual providers who use different electronic health records (EHR) vendors. Provide the staffing expertise to manage EHR implementations for multiple provider sites located in SC, to include start-to-finish eReferral implementation, set-up, go live launch, opt-out process, patient engagement, file transfer protocols, data management, provider outcome reporting, and customized reporting to SC DPH. Provide a telephony or IVR system and SMS (text-based) outreach that engages patients early on in the referral process to increase the likelihood of their enrollment in the Quitline. Interact with Quitline service providers and vendors of certified health records or EHR systems. Possess a working knowledge of how evidence-based tobacco cessation protocols work and have experience in the delivery of effective patient engagement and referral strategies to population-based clientele who are referred by providers from hospitals and other healthcare and treatment systems. Maintain clear and timely communications with SC providers who are engaged in converting from fax referral to electronic referral through their EHR systems. Coordinate provider EHR implementations that have reached their “go-live” date with the SC Quitline Health Systems Optimization Program (HSOP) team to support follow-up recruitment, engagement, training, and technical assistance to help providers better understand the Quitline’s services and systemize electronic referring into standard clinical practice. Provide continuous quality improvement in efficient and effective processes that address low rates of tobacco treatment delivery in healthcare and helps improve tobacco referral rates and overall reach of Quitline services.
