Specifications include, but are not limited to: A. The provider will consult with COC staff and parents of children with severe emotional disturbances to develop survey questions relevant to each perspectives needs. B. The provider will develop a survey instrument to poll families consistently across the state regarding the effectiveness of services considering the following factors as appropriate to the family: 1. Involvement of the family in the childs plan of care 2. Consideration of the familys schedule/convenience in setting meeting times 3. Whether the child is receiving services that are included in the plan 4. Services are put into place to ensure smooth transitions for the child (i.e., related to changes in school, placement, preparing for independent living, etc.) 5. Awareness of services that are available to the child increased with COCs assistance 6. Services and supports for the child improved since COCs involvement 7. Extent of improved access to educational services 8. Degree to which the child and familys underlying needs were addressed 9. Whether the parents skill level and ability to respond appropriately to the childs behavior has improved 10. Relationship between the child and family members improved
