Examples of services may include, but are not limited to: • Services for the elderly to allow them to live independently while receiving appropriate healthcare services in the home • Mental health services provided by psychiatrists, psychologists, social workers, or licensed professional counselors • Substance use prevention and harm reduction services • Identification that a large percentage of residents have a specific healthcare need and need specific related care • Children and youth-focused physical or mental healthcare • Physical and/or occupational therapy healthcare services • Management of chronic disease medical needs • Care management programs for elderly patients facing complex medical needs • Connection and coordination of primary care and other services related to the social determinants of health • Services for people with disabilities that support independent living and community engagement • Events to engage the community and/or create familiarity and rapport with care coordination service team. This may include informational sessions on topics related to the social determinants of health, events for kids and families that include health-related information, bringing special services on site like a flu clinic, etc.
