Specifications include, but are not limited to: Disease Investigation and Partner Service Requirements Provide Disease Investigation, Partner Services, Provider Education and Pre-Exposure Prophylaxis (PrEP) Navigation in order to alleviate the health disparities among Priority Populations in the State. Ensure Disease Investigation and Partner Services: Are available throughout the proposed Region. Target Newly Diagnosed Cases of Human Immunodeficiency Virus (HIV), gonorrhea, and/or syphilis; Prioritizes Cases of chlamydia (i.e. Cases for individuals under fourteen (14) years old and those co-infected with HIV, syphilis, and/or gonorrhea); and Target Cases of known Sexual and/or needle-sharing partners. Provide a sufficient number of Disease Intervention Specialists (DIS) to conduct Disease Investigations, Partner Services, and Clinical Outreach. The DIS shall: Be assigned and responsible for Cases within a specific geographic area. Complete Treatment follow-up, Provider education, and other related services with fourteen (14) days of Case assignment. Ensure Cases are initiated: Within forty-eight (48) hours for HIV and Syphilis; Within seven (7) calendar days for Gonorrhea and Chlamydia. Provide PrEP Navigation for High-risk Negative Cases, including those identified through syphilis and gonorrhea Case follow up; and Referrals Ensure health care Providers are adhering to the U.S. CDC Treatment and testing guidelines. Offer Partner Services to individual Cases. Link the Client and their identified Partner to appropriate health care resources. Ensure testing and Treatment is encouraged as appropriate for individuals identified with Sexual and needle-sharing partners, including offering: Education; Linkage and Navigation on PrEP; Expedited Partner Therapy; and Hepatitis vaccinations
