The Division of Health Promotion is seeking a vendor to provide a diabetes delivery platform to reach all persons who are at high risk for diabetes and enroll them into a lifestyle change program that is led by a fully recognized Diabetes Prevention Program (DPP) delivery organization as designated by the Centers for Disease Control and Prevention (CDCs) Diabetes Prevention Recognition Program (DPRP). The program must be delivered utilizing certified lifestyle change coaches who teach a CDC approved DPP curriculum. This effort is manifested via in-person programs as well as online, virtual environments. We are seeking the following: A custom-branded online platform for delivery of the National Diabetes Prevention Program. A fully integrated online DPP delivery platform that includes CDC approved video curriculum, photo food logging and easy to use coach dashboards, branded mobile apps as well as data collection tools for participant reporting. A single Master License with unlimited sublicensees at no additional cost. Free use of video-based lessons of the online DPP curriculum. Choice of communication tools for coaches to communicate with their participants via conference phone lines, video meeting software, group Facebook pages, email or text. Provide training on how to use the software. Provide technical assistance regarding best practices for recruiting DPP participants for the online program, engaging them and tracking them.
