Specifications include, but are not limited to: • Inspection of SD GFP owned small dams o Inspections shall follow guidelines set forth by Federal Emergency Management Agency (FEMA) and the Association of State Dam Safety Officials (ASDSO). • Provide an inspection report that shall include the following: o Line items described on the checklist form shall be filled out (see attached document). o Visual inspection for surface cracking, erosion, vegetation condition, trees, rutting or puddles, depressions, animal burrows, seepage, slides, scraps, and riprap condition on the upstream embankment, downstream embankment, and the crest of the dam. o Visual inspection for abutment leakage, foundation seepage, sliding, scraping, drainage away from the dam, and hazards in the downstream channel. o Visual inspection noting the type, erosion, vegetation, sidewalls, channel floor, weir condition, energy dispersion, and debris located in the spillway. o Visual inspection of the outlet structure shall be made. The outlet structure shall be inspected for seepage, unusual movement, erosion around the outlet, control mechanisms, and vegetation around the inlet and outlet. If a problem is noted a further in-depth inspection shall be required to gain video or photographic evidence of the problem or failure. o The Consultant shall provide pictures and recommend repairs to the dam. o The Consultant shall provide a cost estimate for repairs to embankment, crest, downstream channel, outlet structure, and spillway of the dam. o The Consultant shall re-evaluate the hazard classification of each dam inspected using aerial photography. Provide documentation justifying the hazard classification • Breach Analysis in accords to Administration Rule “74:02:08 Safety of Dams” that shall provide the following: o A technical memorandum of the breach analysis. o Inundation mapping of the design flow event
