Specifications include, but are not limited to: A.Conduct all inspection operations in accordance with the applicable OSHA safety standards. B.Write a tunnel inspection plan in accordance with section 4.5 of the TOMIE and submit it to the Bridge Management contract manager for approval before beginning inspection. Update the plan after performing the inspection to include any lessons learned, missed items, etc. C.Provide all the equipment necessary to complete the inspections. D.Inspect all requested elements visually. Perform inspections in accordance with FHWA standards. E.Inspect all steel elements and details from a distance not greater than “arms length”. F.Notify MDT immediately of any serious findings that affect the serviceability of the tunnel. Prior to mobilization, request a list of contacts from MDT. If there are critical/imminent structural deficiencies found during inspection that affect structural stability or endanger safety of the traveling public, call down the provided list (and continue re-calling if necessary) until someone is reached live and notified verbally. G.Use photography to document the findings of the inspection. Photograph areas of distress and locations showing the typical condition of the components. MDT requires photo documentation of all condition state 3 and condition state 4 defects. H.In addition to the photography used to represent the tunnel’s condition, the following specific photos are required: •Portal view at each end of the tunnel (2 photos total) •Structure from each street approach crossing over the tunnel (4 photos total) •Each street approach looking from the structure (6 photos total) •Representative view of the underside of the tunnel (this may be multiple photos if the underside has different types of construction or materials) •Load posting signs, if applicable, showing the tunnel in the background I.Enter the tunnel inspection in the MDT Structure Management System (SMS). Inspection entry includes review and updates to inspection data attributes, uploading photos and all other relevant documents, entering repair items, and entering all required elements, defects, photos, and comments. Comments are required for all defects. The consultant will be required to obtain E-Pass credentials through MDT and the Montana Department of Administration. MDT staff will assist with this process. Training on use of SMS will be provided through online services or written tutorials as needed. J.Complete Quality Control processes and submit all data entry, including upload of the inspection report, by routing to the MDT contract manager as directed within 75 days for the inspection. Incorporate MDT review comments and finalize all reports and data within 90 days of the inspection. K.All activities associated with the Tunnel Inspection must be complete and delivered to MDT by December 17, 2020.
