Services that the successful contractor(s) will provide include the following: Personnel will receive the appropriate supervision from within their organization, as well as direction from DPH. Each staff member should have access to certified interpretation/translation services when necessary. It is essential to the success of the program that patients experience a smooth transition from one facet of the program into another, and that the personnel work collaboratively to share information. Bidders must demonstrate capacity to perform the full range of services listed in the Request for Proposal upon signature of the contract and throughout the term of the contract. 1. Proposal Requirements All bidders are required to submit: a. A description of their qualifications and experience, and a minimum of three letters of support. b. A budget and corresponding budget justification. 1) Fringe benefits costs will be limited to 25.44% or less of salary/wages. 2) Indirect costs will be limited to 12% or less of salary total direct costs 3) Health Promotion Advocate salary not to exceed $32,000 per year. 4) Proposals may also include the following budget categories: a) Patient Related Services: patient education, outreach, and other contract specified deliverables b) Non-patient Services: equipment, supplies, professional membership fees, and staff travel and training
