Bidders may be requested to prepare and provide an oral presentation and provide sample materials that demonstrate their proven ability to meet the requirements outlined in the respective Category(s) for which they have submitted a bid. 1. Provide knowledge of data collection and reporting for NQF measures 0018 and 0059.Priority will be given to contractors that can provide documentation in existing ability in utilization of dashboards for data collection and quality improvement initiatives. The dash board must demonstrate having the ability to collect and report on all identified short, intermediate and long-term objectives in the RFP. Provide an example of dashboards in current use. In addition, provide names of all EHR systems your organization is currently working with to demonstrate ability to work with multiple EHR systems. 2. Provide evidence in the ability to work with existing and new practices in quality improvement outcomes for uncontrolled diabetes and hypertension.Priority will be given to contractors that can demonstrate established infrastructure and ability in implementing services for quality improvement in health outcomes among patients with hypertension and uncontrolled diabetes/prediabetes. This includes strong examples of existing ability to work in concurrence with multiple (50+) provider practices, Accountable Care Organizations and other health systems. 3. Demonstrate and foster development and monitoring of Health Information Technology efforts. Priority will be given to contractors that demonstrate examples in existing work efforts with Health IT, education and training, practice/workflow redesign, privacy and security regulations, utilization of evidence based and best practice resources, partnering with state and national health information exchanges (knowledge of multiple types and styles of EHRs) and technical assistance to provider practices (i.e. Meaningful Use, Accountable Care Organizations) and other health systems in the state. 4. Provide comprehensive monthly progress reporting to the Diabetes and Heart Disease Prevention and Control Program. Priority will be given to the contractors who can show current ability to provide efficient and adequate capabilities in reporting on status and performance measures on projects, activities and progress reports. The vendor will communicate with staff from the Diabetes and Heart Disease STATE OF DELAWARE Delaware Health and Social Services, Division of Public Health 7 Prevention and Control Program on a regular basis and should demonstrate good processes for communication skills
