State intends to enter into an agreement with the Intended Source for on-site music therapy for consumers residing at State. Services will be approximately one day per week and will be held at various locations on the State campus. Intended Source will provide all necessary equipment to provide therapy services. Services to be included are: individual assessments as requested, 1:1 individual 30-minute therapy sessions, 2:1 group therapy sessions, small group therapy sessions providing services to 4-5 consumers together, medium group therapy sessions providing services to 6-8 consumers together, large group therapy sessions providing services to 8-10 consumers together, and staff training as requested. Intended Source shall provide one hour per week after therapy is completed to document consumers’ progress and plan for upcoming sessions. No additional specifications were given.
