Specifications include, but are not limited to:Proposed Methodology and Work Plan This section shall describe in detail the approach that will be taken to carry out the activities described in the Scope of Services section of this RFP. Specific completion dates for the various tasks must be shown. The work plan shall outline specific objectives, activities and strategies, and resources. 1. Applicant must specify the date the proposing firm was organized to provide clinical laboratory services in/to healthcare facilities. 2. Applicant must provide documentation that the laboratory facility which is proposed as the provider of services to DPC is Accredited by the College of American Pathologists (CAP). 3. Applicant must provide a brief history of the organization responding to the Request for Proposals. Please include a description of the management structure, current staffing, current services provided by the organization, target populations served, and any other information pertinent to demonstrating the organizations capability to provide the required services. 4. Applicant must specify organizational experience in providing clinical laboratory services. Applicant must also include in the discussion the number of employees in the organization, annualized dollars of payroll, and number of years in business. 5. Applicant must provide a corporate organizational chart that clearly identifies where in the organizational structure the proposed services will be placed.
