Specifications include, but are not limited to: Furnishing all supplies, materials, tools, equipment, labor, personnel, and supervision; pay all taxes, insurance, bonds, license and permit fees, and all other direct and indirect costs necessary to provide as-needed Blood Testing Services. a. Provide upon request from the contracted physicians listed on Exhibit A-1 Physician and Clinic Locations, or CHP Project Representative, (Collection Centers), the collection kit that shall consist of all materials normally used in blood collection procedures. b. Arrange for overnight courier service to pick up and deliver the collected specimens from collection centers to the contractor’s lab. The specimens shall be placed in containers designed to minimize the possibility of damage during shipment. c. Contractor shall be responsible for any expense related to overnight courier services, postage costs, laboratory reports, and all other direct and indirect costs associated with this Agreement.
