Specifications include, but are not limited to: As part of this Grant, DPBHS is creating a statewide Wraparound Case Management Team to work with children ages birth to 18 that are receiving services from both DPBHS and the Division of Family Services (DFS), which is also located within DSCYF. The goal of this team is to provide comprehensive, integrated family care for multi-stressed children and youth to: 1) increase positive family/caregiver interactions, 2) facilitate connection with natural, community supports, and 3) promote parent/caregiver empowerment to navigate systems. DPBHS is seeking proposals from qualified individuals to staff this team, including 1 licensed Clinical Team Leader and 2 Clinical Service Coordinators.
