Specifications include, but are not limited to: The OTR/L will determine and administer appropriate evaluation and re-evaluation tools, develop individualized treatment plans in order to maintain and improve client functioning and quality of life, observe and record client progress, and make subsequent relevant recommendations and modifications of occupational therapy goals, as necessary, while following all specified documentation, communication, and treatment protocols. Clients who display symptoms that might suggest the need for OT evaluation will be identified by the DPC Recovery Team. When a person with possible need for OT evaluation is identified, a formal request for OT evaluation will be generated by the clients Recovery Team. A representative of the Recovery Team will then contact the OT, to schedule the assessment. Following receipt of the request, the OT will determine and administer appropriate evaluation tools and a written report should be provided to the referring Recovery Team. Included in the OT evaluation report should be general findings related to the clients level of functioning, including whether deficits were identified and, if so, what treatment is needed to appropriately address those deficits. The report should indicate whether treatment will need to be provided solely by the OT, or by DPC OT Aides and OT Assistants, or if the OT will train other DPC staff to provide needed treatment to the client. The report should include a specific treatment plan, indicating specifically what treatments are to be provided, by whom, and at what frequency.
