A. Services and all requirements as stated below and as required by law shall be provided by the Contractor for the contracted services, at the request of the Contract Manager, or designee. The services shall include, but are not limited to the following: 1. Provide oral surgery services under local anesthesia at the Contractor’s licensed facility to treat residents referred by the attending Medical/Dental staff of the Home. Oral Surgery services provided to residents shall be equal to or higher than Denti-Cal standards and requirements. Treatment under oral/conscious sedation and general anesthesia will not be covered by the Home and would be the financial responsibility of residents, unless previously pre-authorized by the Contract Manager, or designee. 2. Provide consultation and assessment, treatment planning and treatment follow-up as necessary. 3. Ensure that there is continuity of care for the residents of the Home during periods of absence by the Contractor’s primary provider. 4. Comply with Prior Authorization for Treatment requirements as stated in Exhibit A, section 5. 5. Render oral surgery services to any Home resident referred for pre-authorized dental care services/treatments and assumes full responsibility for the provision of those services to residents for which a complete medical history and pre-existing conditions were provided by the Home. Contractor shall not be held fully responsible for pre-authorized services/treatments provided if the Home fails to provide advance written medical history and notification of pre-existing conditions to the Contractor prior to services being rendered. 6. Maintain all dental records in the required manner in accordance with dental professional standards and Denti-Cal requirements or greater. All records will be the property of the Contractor. The Home will be provided copies of dental records upon submittal of consent form to release information. 7. Provide the Home’s attending physician the customary dental records including, but not limited to, progress reports and clinical summaries, if applicable. Contractor recommended patient plans of care shall be included in each resident’s progress report or clinical summary.
