1. Provision of Telemedicine • Provide a clear workflow outlining process and procedure, including: ▪ Process for on-scene activation. ▪ Number of patients that may be served at any given time. ▪ Physician/Provider types, specialties and qualifications. ▪ Average length of telemedicine assessment. ▪ Process for patient consent to telemedicine assessment. ▪ Process for handling patients who may require higher level of care. ▪ Process for communicating plans of care to community providers. ▪ Process for prescribing medications to patients. 2. Provision of Care Navigation • Provide a clear workflow outlining process and procedure, including: • Process for any provided care navigation during or after telemedicine assessment by physician and/or provider. • Process, if provided, for transportation assistance for follow-up non-emergency care plan referrals. • Process, if provided, for delivery of prescriptions for medications or durable medical equipment ordered during telemedicine assessment. • Process, if any, for coordination of any referrals/orders facilitated during telemedicine assessment for home care, hospice care or medical care. 3. Egalitarian Access to Care • Process, if any, for equal provision of telehealth assessment for all medically appropriate referred patients regardless of race, religion, gender, language, culture, political affiliation, disability, socioeconomic status, health insurance coverage or lack of health insurance coverage, and income. 4. Compliance • With the understanding of all federal and state collection laws, HIPAA of 1996, and applicable Florida statutes, what information can be provided on the following: • Provide information on how patient information is collected and health information is stored. • Provide information on how collected patient information and health information is shared.
