A. MAINTENANCE & INSPECTION SERVICES (Quarterly) Equipment Description Traffic Light Post with Street Light Fixtures Five (5) Light Towers Three (3) Light Towers $ - $ - CONTRACTORS RESPONSIBILITIES The Contractor shall perform, on a quarterly basis, a detailed check for proper operation of controller assemblies and signals. The Contractor shall provide the requested services on the dates specified by the Institution Contract Liaison or designee at mutually agreed upon times by the Institution Contract Liaison or designee and the Contractor. Services shall be performed during the designated business hours 7:00 a.m. to 3:00 p.m., Monday through Friday, State holidays excluded. CHCF reserves the right to request additional services at the Unit rate specified in Exhibit B-1, Rate Sheet, or to decrease the scheduled number of services, should equipment be added, or old equipment becomes obsolete. The Contractor will be notified in writing by the Institution Contract Liaison or designee of equipment that has been removed from the list. There are a total of four (4) Traffic Light Posts with Street Light Fixtures, one (1) Traffic Light Cabinet, two (2) Five Light Towers which include two (2) Green Arrows and two (2) Yellow Arrows, and twelve (12) Three Light Towers. A detailed check shall be made at 90-day intervals and should include the following items as a minimum: (A) Field Inspection 1. Visual check of indications. 2. Vehicle and pedestrian head alignment. 3. Hardware (hand-hole covers, signs, poles, backplates, etc.). 4. Pullbox covers (broken, missing, and clear of dirt or debris). 5. Condition of street name signs. 6. Visual check of service cabinet and equipment locks. 7. Traffic handling of intersection. 8. Visual check of loops in the roadway.
