A. Provide qualified personnel to provide the STATE’s EIS with POCM services, other ad hoc project support services (e.g., HEIDS testing and training supports), and technical assistance. The CONTRACTOR’s personnel shall be knowledgeable of IDEA Part C 34 CFR Part 303; HRS §321-351 to 321-357; HAR, Chapter 11-140, EI Services for Infants and Toddlers; and Hawai‘i IDEA Part C EI Policies and Procedures and understand the data collection and reporting requirements for federally mandated EI services. Additionally, the CONTRACTOR’s shall develop an understanding of the STATE’s EIS business and technical requirements of the HEIDS project to ensure that the go-live readiness and preparation of the HEIDS project is able to be deployed successful. The CONTRACTOR and the CONTRACTOR’s personnel shall be responsible for delivering the required services and ensure that the deliverables are met in accordance with this Scope of Work (“SOW”). 2. The contracted services shall be performed via telework unless requested and authorized by the STATE’s EIS for onsite in-person services. 3. The CONTRACTOR is permitted to use subcontractors to perform any of the work or services a part of the SOW with prior approval by the STATE. The CONTRACTOR will be responsible for any agreements with the subcontractor. The STATE and the STATE’s EIS is not agreeing to and is not responsible for any terms and conditions that the CONTRACTOR has with any subcontractor. The CONTRACTOR shall be solely responsible for performance of the entire contracted services whether or not subcontractors are used. Any issues that arise as a result of this relationship shall be resolved by the CONTRACTOR. All references to the CONTRACTOR shall be construed to encompass both the CONTRACTOR and any of the CONTRACTOR’s subcontractor(s). B. Provide POCM services which shall include but not limited to: 1. Provide objective third-party feedback, recommendations, monitoring and project oversight of the HEIDS project vendor(s)’ progress, compliance, and performance to verify and validate database system(s) go-live readiness and product deployment. This includes ensuring that the database system(s) meet planned features and functional requirements in accordance with the STATE’s CSHNB requirements and the applicable database system SOW(s) 2. Assist the STATE’s EIS in mitigating risks and issues through risk management activities, which may include, but are not limited to, scheduling and facilitating risk management meetings; identifying, tracking, and escalating risks and issues; and providing recommendations and a structured approach to address and close risks, issues, and other project concerns. 3. Assisting the STATE’s EIS with maintaining and managing project logs (e.g., risk and issue log, action item log, decision log) and integrated HEIDS project work plan/schedule, making any recommendation for change orders to the current HEIDS Project Contract, as applicable and/or assessing whether the database system(s) have the capability to support future enhancements (e.g., changes that fall outside the scope of the current SOW(s) or contract(s)), when such changes are identified and prioritized. 4. Assessing the go-live plan and advising any potential changes to help ensure a successful go-live for system users which may include but not limited to assess the effectiveness and advise on the design, execution, and any changes that might be needed to the go-live support model and developing operational readiness criteria that the STATE’s EIS can use to decide whether to proceed to the go-live milestone of the HEIDS.
