STATE is looking for Proposals designed to improve quality of life, promote equity, and foster increased community participation for people with disabilities in Minnesota under one or more of the services listed above (housing, employment, transportation and culturally responsive services). Priority will be given to Proposals with innovative ideas that: Serve people with disabilities who are eligible for Home and Community Based Services (HCBS), and those services are not already paid for through a waivered service. Conduct outreach to underserved populations. To provide, “locally focused”, “individually centered”, and “easily accessible” services, conduct outreach activities to historically underserved populations and/or communities where health inequities exist. Address the needs of all people with disabilities including people of color, Native Americans, Asians, Latine, people who have a primary language other than English, transitioning young adults, undiagnosed individuals with complex behavioral and mental health needs, and other diverse groups. Develop training plans, and train staff to provide person-centered and culturally responsive services to recipients in underserved communities (e.g., workshops, certifications, or courses for staff). Promote coordination or collaboration with system partners such as, but not limited to, schools, Vocational Rehabilitation Services, lead agencies, providers, and community organizations. Develop sustainability and scalability – demonstrate the project could be replicated or expanded and used across Minnesota after the grant term ends.
