The Bidder shall be responsible for the following tasks and responsibilities: Work in collaboration with the DOH to identify, investigate, and resolve barriers to improve the collection and accuracy of fetal death data; Implement a fetal mortality review (FMR) team, with focus on data collection, case review, community action, and system change; Establish a team to collect and review de-identified case information, including maternal interviews and medical records; Create a multi-disciplinary Case Review Team (CRT) to analyze the cases and identify contributing factors and system-level problems; Involve a Community Action Team (CAT) – a multidisciplinary group to review cases, identify trends and root causes, and then develop actionable recommendations to improve maternal and infant outcomes. The team should include diverse stakeholders and follow a structured review process to ensure comprehensive analysis, with a focus on preventing future deaths through system-level changes; Monitor and evaluate the FMR's impact to ensure ongoing quality improvement and evidence-based interventions and policies to improve maternal and infant health outcomes; Collaborate with DOH, obstetrician/gynecologists, midwives, and hospital administration to coordinate efforts and share critical information. Effectively communicate findings, recommendations, and best practices; Document the process, supports, and considerations for the implementation of the fetal death review; Document and discuss recommended updates to the reporting system, data collection and accessibility, and any evidence-based recommendations to policy changes related to fetal death reporting, prevention strategies, and healthcare practices to (DOH); Provide DOH with regular monthly updates on progress, monitor outcomes, and identify areas where further advancements are needed; and Ensure all tasks, deliverables, and activities have been completed and submit a Final Project Report to (DOH) within thirty (30) days after the end of the contract period.
