i. Provide toxicology testing and “Chain of Custody” procedures to account for the integrity of each urine specimen by tracking its handling and storage from point of specimen collection to final disposition of the specimen. ii. Provide the approved “Chain of Custody” form that shall be used for maintaining control and accountability of each specimen from the point of collection to final disposition of the specimen. iii. Provide upon request, from the contracted physicians listed on Attachment 1, Physician and Clinic Locations - PSO, PSD, and Cadets, and Attachment 2, Physician and Clinic Locations – Non-Uniformed, Non-Sensitive, and Sensitive Classifications, or CHP Project Representative, the collection kit that shall consist of all materials normally used in specimen collection procedures, i.e., urine container, seal for container, temperature strip, label, and packaging containers and supplies. iv. Provide shipment of collected specimens to the drug testing laboratory using overnight courier services. The specimens shall be placed in containers designed to minimize the possibility of damage during shipment.
