The work shall be performed as specified in this Scope of Services and is generally described as supplying all labor, equipment, materials, and forces necessary to complete mobilization, demolition and removal, excavation and backfill, well casing extension, drain rock placement, concrete and base installation, surveying, and cleanup. This project generally consists of extension of the 4-inch schedule 40 steel well casing and replacement of the 2-foot x 3-foot vault over Monitoring Well FVM-1 located in Mile Square Park in Fountain Valley. The scope of work will generally include, but not be limited to, the following: 1. Mobilize to the Site; 2. Demolish and remove the existing vault; 3. Excavate the area around the well casing to a minimum of 2-feet below the top of the existing casing; 4. Weld a two-foot casing extension, consisting of pipe that matches the material and diameter of the casing, onto the top of the well casing, and protect in place 1.75-inch diameter PVC liner; 5. Install 3/4-inch crushed aggregate base, compacted to 90% compaction, with minimum dimensions of 5-foot square and 24-inch deep, centered on the well casing; 6. Install a new Jensen K2436-FH33-08T vault with lid; 7. Backfill with native material to concrete subgrade with minimum compaction of 90%: 8. Place 10-foot square, 4-inch thick Aggregate Base, compacted to 95%, 9. Place 10-foot square, 6-inch thick welded wire mesh reinforced concrete maintenance pad; 10.Utilize a registered surveyor to survey the elevation of the top of the finished casing and top of finished vault. 11.Maintain site during construction and perform final site clean-up and restoration. 12.Provide all records, as required.
