A. Contractor Requirements: The Contractor shall furnish all qualified technical personnel, supervision, equipment, and materials as necessary, for Mobile Imaging Services in accordance with the terms, conditions, provisions, currently accepted professional standards, all applicable federal, state and local laws and regulations and schedules set forth herein, seven (7) days per week, twenty four (24) hours per day. / 1. Contractor and the contractor’s personnel shall not be currently suspended, debarred or otherwise ineligible to participate in federal/state health care programs, including Medicare and Medicaid programs. 2. Contractor shall supply a copy of license to perform Medical Imaging in the Commonwealth of Virginia with their response to this proposal. 3. The Contractor shall maintain and provide (upon request) an up to date list of all personnel performing work under this contract with written evidence of the personnel’s qualifications, license and certifications. Vendor shall supply to the Care Center, at no charge to the Care Center, upon written request, qualifications, license and certifications on any personnel providing Medical Imaging to the Care Center. 4. Contractor personnel performing Medical Diagnostic Services for the Care Center shall have an up to date criminal history/background check that, upon written request, shall be provided to the Care Center at no charge. Contractor personnel shall not have been convicted of any “Barrier Crimes” as identified by Virginia statute. 5. The Care Center reserves the right to reject services from Contractor’s personnel deemed by the Care Center to be unqualified, disorderly, or otherwise unsuitable or unable to perform assigned work. Refusal of any of Contractor’s personnel shall not be based on race, color, religion, sex, age, national origin, disability or political affiliation in accordance with Equal Opportunity Employment Guidelines. The rejected personnel shall be immediately removed and prompt arrangements made for a suitable replacement. 6. The selected Contactor is not employed by the Care Center, but is engaged as an independent contractor. The selected Contractor will indemnify and hold harmless the Care Center and its employees and agents, with respect to all withholding, Social Security, unemployment compensation and all other taxes or amounts of any kind relating to the selected vendors performance of this agreement. Nothing in this agreement will be construed as authority for the Selected Vendor to make commitments which will bind the Care Center, or to otherwise act on behalf of the Care Center. 7. The Selected Contractor shall comply with all federal or state laws and regulations pertaining to Worker’s Compensation Requirements for insured or self-insured programs.
