Case Review and Data Collection: 5.2.1. Complete case reviews for sudden unexpected infant deaths (SUID) within three (3) months of identification. Enter information into the national database within thirty (30) days of the review of the SUID and send a complete copy of the SUID case file to the Child Fatality Program Manager within one (1) week of the completed review. 5.2.2. Complete case reviews for abuse and neglect deaths. Enter information into the national database within thirty (30) days of the review of the abuse and neglect death and send a complete copy of the abuse and neglect case file to the Child Fatality Program Manager within one (1) week of the completed review. 5.2.3. In collaboration with ADHS, local law enforcement and medical examiners, identify and request records related to all child deaths within the service area. 5.2.4. Conduct timely reviews of each child death, gathering information from all relevant sources, annually by June 30th. 5.2.5. Accurately and completely enter all case data into the ADHS-approved child fatality review national database. 5.2.6. Respond to data quality checks from ADHS within one (1) week. 5.3. Reporting: 5.3.1. Prepare the Team's quarterly reports by completing the quarterly report form in its entirety and submitting to the Child Fatality Review Program manager by the 15th of the month following end of quarter. 5.3.1.1. Quarterly reports should include descriptions of how many cases have been assigned at that time, how many reviews have been completed, how many review meetings have been held, any changes to the roster, and any other successes, challenges, or implementation of prevention recommendations the local coordinator wishes to share.
