A. Scope of Services: 1. Sub-contract and/or provide psychoeducational, trauma informed, personcentered in-person support groups (IPSG) for people living with TBI, their families, and caregivers. 2. Sub-contract and/or provide person-centered community integration activity (CIA) to people living with TBI, their families, and caregivers. 3. Sub-contract and/or provide peer-to-peer mentoring and navigation (PTP) services. 4. Provide training and networking mechanisms for TBI support group facilitators of IPSG’s, CIA’s and PTP’s. 5. Collect, analyze, and report program data, including but not limited to: a. Number of programs implemented, and geographic regions served by county. b. Demographics of participants and facilitators, including global majority populations, LGBTQIA individuals, veterans, etc. c. Participant and facilitator satisfaction survey results. 6. Quantitative and qualitative measures showing incremental improvements in recovery outcomes, TBI awareness, and participant knowledge. a. Program cost analysis including expenses directly and indirectly associated with development, implementation, and maintenance of programs. b. Monitor and assess sub-contractor(s) to identify address service gaps or barriers, ensuring programs reach intended populations and meet quality standards. c. Provide program support, technical assistance, and training as needed to promote continuous improvement. 7. Share best practices, evidence-based, and culturally responsive training. 8. Implement marketing and public awareness initiatives to promote all program offerings. 9. Work with TBI Program Manager to coordinate outreach with other TBIfunded programs, DSHS, and state agencies. 10. Ensure promotional efforts reflect diversity, equity, and inclusion goals and support increased engagement and access to all populations. This includes developing culturally and linguistically appropriate materials and building partnerships with local organizations, community leaders, and service providers. 11. Report monthly to TBI Program Manager with program data, progress, and delivery. Provide a year-end report to TBI Program Manager that includes annual data and analysis of program delivery.
