A. Contractor shall provide the following services: 1. Schedule physicals within seven (7) calendar days of referral. 2. Request picture identification from prospective employee before services are performed. 3. Contractor shall be physically located within a thirty (30) mile radius of the home. 4. Contractor shall review Job Description/Duty Statement to assess the prospective employee(s)’ ability to perform the essential functions of the position. 5. Contractor shall provide Basic Pre-Employment Physicals to include: medical history, height, weight, blood pressure, oral temperature, vital signs, vision, and musculoskeletal assessment. 6. Contractor shall provide Annual Employment Physicals to include: medical history, height, weight, blood pressure, oral temperature, vital signs, vision, and musculoskeletal assessment. 7. Contractor shall provide Tuberculosis (TB) Skin Type Test Step-1. 8. Contractor shall administer a 2-step Tuberculin Skin Test (TST) and/or X-ray, if current or prospective employee(s) are not able to comply with the skin test procedure or need further diagnostics. 9. Contractor shall complete and return a Health Screening Report, LIC503, Interim California Department of Human Resources (CalHR) Pre-Employment Health Medical Report, and 6470 TB Screening and Questionnaire and a Placement Physical Examination governed by the California Department of Social Services/Community Care Licensing Division (CDSS/CCLD) to the assigned Contract Manager. 10. Contractor shall submit current or prospective employee’s examination results immediately to the Contract Manager via facsimile (661) 974-8189 along with an invoice. 11. Contractor shall submit employee’s original examination results (LIC503) within seven (7) calendar days via US mail, in a sealed envelope marked “confidential”, to the CalVet Human Resources Division Contract Manger’s attention. 12. Contractor must NOT attach (via facsimile or mail) a LIC503 Form with the “Original” Invoice to Accounting for payment. LIC503 Form is a “Confidential” document. 13. Contractor shall submit a complete service report to the Contract Manager after completion of service. The report shall include: date of service, description of service provided, name and signature of service technician, and signature of State employee certifying indicated services were performed. B. Contractor shall offer the Vaccinations (not mandated but required to offer): 1.Hepatitis B 2.Influenza 3.MMR (measles, mumps, and rubella) 4.Tdap (tetanus, diphtheria, and pertussis) 5.Varicella
