Battery(s) make, model and total capacity in cold crank amps (CCA) Transmission make, model and serial number. If so equipped, chassis transmission PTO(s) make, model and gear ratio Pump make, model, rated capacity in gallons per minute and serial number Pump transmission make, model, serial number, and gear ratio Auxiliary pump make, model, rated capacity in gallons per minute (liters per minute where applicable) and serial number Water tank certified capacity in gallons Paint manufacturer and paint number(s) Company name and signature of responsible company representative Certification of slip resistance of all stepping, standing, and walking surfaces. If the apparatus has a fire pump or an industrial supply pump, the pump manufacturer's certification of suction capability. If the apparatus has a fire pump or an industrial supply pump, a copy of the apparatus manufacturer's approval for stationary pumping applications.
