A. Purchase a Nationwide License for the Stress-Busting Program for Family Caregivers from WellMed; B. Train and maintain the required number of Facilitators; 8 i. Provider Agencies awarded two (2) or more grants shall train a minimum of two (2) unique Facilitators per award; 1. Provider Agency shall maintain a minimum of two (2) unduplicated Facilitators per award throughout the grant period. ii. Provider Agencies awarded only one (1) county shall train a minimum of three (3) Facilitators; 1. Provider Agency shall maintain a minimum of three (3) Facilitators throughout the grant period. C. Offer Stress-Busting classes outside of regular business hours (M-F 8 a.m.-5 p.m.); D. Utilize DoAS resources to assist Stress-Busting class participants to receive respite care for the PLWD in their care when they attend classes; i. Agencies are encouraged to work with Adult Day Centers that are willing to provide care to the PLWD while their caregiver attends the Stress-Busting class. E. Attend and participate in routine meetings/calls with DoAS and partnered StressBusting agencies, as required by DoAS; F. Submit quarterly fiscal and programmatic reports via the State’s System for Administering Grants Electronically (SAGE) as required by DoAS; i. A written justification as to how the funds were spent for each class series completed during the reporting period, shall be provided alongside quarterly fiscal reports; and ii. Reports shall be subject to verification through accompanying programmatic reports and independent review by DoAS to confirm alignment with the project deliverables. Expenditures that cannot be verified will not be approved for reimbursement. G. Ensure advertisement of the Stress-Busting Program is limited to New Jersey for the duration of the grant period; H. Ensure a minimum of $25,000 is spent on classes throughout the grant period; and i. Funding for in person and virtual classes will be allocated as a Stress Busting class series package, as stated in the Scope of Work. I. All successful applicants who are not governmental entities are required to register with NJSTART, the State of New Jersey’s eProcurement system, which enables agencies to be paid.
