Provide housing, housing assistance and supportive services to individuals and/or families experiencing homelessness, through Transitional Housing (TH) (henceforth referred to as TH Programs or Programs). Proposers may propose providing any or multiple TH Programs. Individuals receiving the Programs’ services will hereinafter be referred to as Participants. Programs will serve individuals and/or families experiencing homelessness who are at elevated risk of harm if they remain unhoused, including increased risk of experiencing a mental health inpatient stay, jail booking and/or frequent use (4 or more visits) of hospital emergency rooms. All participants will be referred to Programs through the CoC’s Coordinated Entry.
