For individuals who are between thirteen through seventeen years of age, permission from a parent/guardian must be required. 2. A digital self-assessment featuring a comprehensive clinical evaluation that integrates validated clinical questionnaires, along with relevant personal context. a. Based on digital assessment, a personalized plan will be developed for the user b. Opportunity for the assessment to be completed every so often to ensure the plan best meets the user’s needs. i. The plan and content are continuously refined as the user engages with the program on an ongoing basis for measure-based care. 3. A comprehensive digital library, available to the user in addition to the personalized plan, which includes: a. Searchable content featuring videos, articles, and exercises curated based on the user’s needs and preferences as determined by the assessment. b. Evidence-based digital courses that guide the user through the curriculum. i. Wellness and health promotion areas: 1. Healthy weight (BMI) maintenance 2. Smoking and tobacco use cessation 3. Encouraging physical activity 4. Healthy eating 5. Managing stress 6. Avoiding at-risk drinking 7. Identifying psychiatric symptoms through self-assessment 8. Recovery and resiliency 9. Treatment monitoring ii. Evidence Based Information 1. The organization meets the requirement of “evidence-based” information if recognized sources are cited prominently in selfmanagement tools. If the organization’s materials do not cite recognized sources, the organization is able to produce a documented process detailing the sources used and how they were used in developing the self-management tools.
