All services are to be provided according to the following schedule: o Laboratory-related items: Monday through Friday 8:15 am – 2:00 pm o Pharmacy-related items: Monday through Thursday, 8:15 am-2:00 pm o No services are to be provided when the DKPH is closed for state holidays or other announced closings, such as inclement weather. 2. Laboratory-related pick up will be pre-packaged for transport and will occur from the following locations: o E.L. Richardson Health Center, 445 Winn Way, Decatur, GA 30030 o T.O. Vinson Health Center, 440 Winn Way, Decatur, GA 30030 o Clifton Springs Health Center, 3110 Clifton Springs Road, Decatur, GA 30034 o East Health Center, 2277 So. Stone-Mountain-Lithonia Road, Lithonia, GA 30058 o North Health Center, 3807 Clairmont Rd. NE, Chamblee, GA 30041 Room# 1128 3. Delivery of laboratory-related items, such as clinical specimens, will occur before 1:00 p.m. EST to the State of Georgia Public Health Laboratory, located at 1749 Clairmont Rd., Decatur, GA 30033 4. Pharmacy-related items will be picked up from DKPH, in the provided storage containers(s), and delivered to the following health centers: o E.L. Richardson Health Center, 445 Winn Way, Decatur, GA 30030 o T.O. Vinson Health Center, 440 Winn Way, Decatur, GA 30030 o Clifton Springs Health Center, 311O Clifton Springs Road, Decatur, GA 30034 o East Health Center, 2277 So. Stone-Mountain-Lithonia Road, Lithonia, GA 30058 5. All storage containers will be returned to DKPH, as needed
