The proposer must demonstrate capacity to provide, at minimum, the following services at the Municipal Clinic Building located at 218 N. Broad Street, First Floor, Trenton, NJ, and through community-based encounters throughout the City of Trenton, such as with mobile units or outreach events, as approved by the City: • Adult and pediatric primary medical care (office-based). • Chronic disease management (diabetes, hypertension, asthma). • Preventive services: immunizations, vaccination of children, school, sports, and work physicals, cancer screening referrals, and well visits. • Sexually Transmitted Disease (STD) testing and treatment services for City of Trenton residents unable to pay. The facility must provide both diagnostic testing and treatment for STDs. • HIV testing services for City of Trenton residents unable to pay. (Note: testing only; treatment services are not included. The facility must make referrals for HIV treatment and care coordination.) • Lead testing and reporting in collaboration with the City’s Department of Health & Human Services Health Promotions Unit, ensuring timely data submission and compliance with all public health reporting requirements. • Community-based services via mobile unit or approved outreach events, including vaccination drives and screenings for HIV, STDs, and cancers such as prostate, skin, and breast cancer. • Collaboration with the City’s Department of Health & Human Services and other agencies for referrals to social and mental health services to ensure continuity of care. • The facility will share aggregate patient data in a mutually agreed-upon format to comply with the City’s requirements for any of its federal and state reporting requirements. • The facility will provide all outpatient in-office services to City residents without charge to such patients if they are unable to pay for a visit. • The facility will work collaboratively with the city on disease prevention and health promotion activities, which should be directed to specific populations, including the City’s schools. Services shall include, but not be limited to, awareness of HIV testing and treatment referral services, senior care, nutrition, and wellness education programs. • The facility will provide health care for individuals infected with HIV or who are suffering from AIDS through appropriate referrals and coordination of care. • Data reporting: monthly aggregate patient counts, service types, payer mix, and demographic summary in a mutually agreed digital format.
