The course should be delivered through a combination of in-person sessions and interactive virtual sessions, featuring experiential, hands-on activities and assessments that reinforce learning. Participants who successfully complete the course will be eligible to receive a certificate of completion. The contractor will also collaborate with the CoC Planning Team to digitize the course for future on demand access. Core topics should include, but are not limited to: • Case Management Fundamentals: The complete case management process, including the intake, assessment, initial service planning, implementation of the service plan, proper documentation of client interactions, evaluation of progress made, and revising service plans based on progress. • Trauma-informed care: Strategies for applying trauma-informed principles when working with clients, addressing the intersection of mental health disorders, substance use disorders, and homelessness. • Behavioral health training: Approaches for working with high acuity clients, including how to refer clients to behavioral health service providers and collaborate with them. • Client-centered case management: Emphasis on maintaining confidentiality, avoiding conflicts of interest, and respecting client choice. • Meaningful client engagement: Techniques for building rapport and trust with clients through consistent, transparent, and respectful communication. • Working with Subpopulations: Guidance for effectively serving specific groups, including young people ages 16 – 25, survivors of domestic violence, veterans, and others. • Collaboration and Stakeholder engagement: Building strong relationships with service providers, community partners, and other stakeholders to coordinate care. • Housing Stability strategies: Approaches for helping clients, especially those in time limited programs such as Rapid Rehousing, maintain stable housing. • Eviction prevention practices: Tools and strategies to identify and mitigate risks that could lead to eviction...
