The City seeks a verifiably qualified professional, an organization, or a combination of verifiably qualified professionals to assist the City in creating a long-term effort to improve community health. The contractors will be tasked with leading the REACH efforts, in partnership with the Office of Planning & Development- Community Development Department, and the Mayor’s Health Task Force’s Healthy Active Living Working Group (HALWG). This work requires cross sector partnerships relevant to specific strategies (e.g., transportation, charitable food system, housing, parks and recreation, preparedness, healthcare, education, or other sectors). Applicants are encouraged to use a community-based participatory approach that builds on existing community assets and existing coalitions, allowing for the flexibility necessary to tailor interventions that meet the unique needs of our residents. All proposed activities must use local level policy, system and environmental (PSE) approaches that support community level nutrition, physical activity, and breastfeeding strategies. All proposed activities must be grounded in health equity and aim to reduce health disparities and address factors that influence health. The Contractors will lead programmatic efforts and with support from the HALWG, and coordinate with the MHTF and REACH Coordinators, and/or the Community Development Dept. Director for review and authorization to proceed. The work will be conducted in any of the following program categories, in which the applicant is most comfortable: • Nutrition (1.1): Implement local level policies and activities that promote food service and nutrition guidelines in facilities, programs or organizations where food is sold, served and distributed. • Nutrition (1.2): Implement local level policies and activities that coordinate the uptake and expansion of existing fruit and vegetable voucher incentive and produce prescription programs. • Physical Activity: Implement local level policies and activities to connect pedestrian, bicycle, or transit transportation networks (e.g., activity-friendly routes) to everyday destinations. • Breastfeeding: Implement local level policies and activities that achieve continuity of care for breastfeeding families. The Contractor is required to prepare a planning timeline, inclusive of a work plan and schedule for authorized grant activities and reporting requirements. The Contractor will be expected to consider public health in its broadest context, centered on addressing health inequities and the social determinants of health, and inclusive of the built environment. Additionally, the applicant will be expected to support project implementation, with a focus on rigorous evaluation and data collection practices that measure progress, community engagement, and equity impacts. He/She/They will be expected to: • Promote adoption of food service and nutrition guidelines in local facilities, programs, or organizations. • Increase community participation in fruit and vegetable voucher incentive and produce prescription programs. • Facilitate the development of activity-friendly routes connecting residents to key community destinations. • Strengthen systems of care to ensure consistent breastfeeding support across settings. • Track community-level data aligned with the focus areas and the social determinants of health (SDOH). • Meet with CDC Project staff monthly • Meet with local REACH Project partners monthly, and • Provide regular, timely reports to the City
