1. Provide 12 separate residential spaces for mothers with up to three children each to support family preservation or reunification efforts based on referrals received from Health and Human Services. 2. Provide staff or approved supervision at all times in the facility to monitor and ensure the facility is safe for children and residents and there is no substance use in the facility. 3. Provide transportation options to support program participants' attendance at substance use treatment or other mental health/ parenting services addressing their needs as identified by HHS or the courts. 4. Attend a monthly "treatment team" meeting composed of Contractor and BCHHS staff to discuss case trajectory. Attend Child and Family Team meetings to discuss program participant's progress, when needed. 5. Submit a quarterly report indicating entries into program, exits from program, reason for discharge, type of services/ treatment being provided. Include all residents for the time period. Submit an annual report indicating effectiveness of program for participants served. 6. Provide a case manager to work with each mother using evidence based treatment modalities when possible and sound clinical practice. Examples of practices used by Contractor include Motivational Interviewing, Trauma Informed Care, Recovery, Substance Abuse certification and Triple P Parenting.
