The PROVIDER shall deliver Intensive Case Management Plus (ICM+) services to individuals identified as high utilizers or experiencing homelessness, with a focus on person-centered, trauma-informed, and recovery-oriented care. Services shall include individualized community-based case management, treatment coordination, psychosocial rehabilitation and peer support (as applicable) improve housing stability, reduce hospitalizations and justice involvement, and promote engagement in treatment. ICM + High Utilizer are services provided to consumers who have experienced recurring crises, frequent hospitalizations or incarcerations within a six (6) month period, and who require a higher level of service coordination to achieve and maintain stability in their housing placements ICM + Homeless are services provided to consumers experiencing homelessness who are frequently arrested, and subject of repeated MH-1 calls and have high utilization of emergency department services.
