The required services to be provided are for as-needed heating, ventilation, air conditioning (HVAC) and boilers as needed repair services at Lubbock State Supported Living Center (LbSSLC). Contractor shall perform, as requested and approved by the facility, services at Lubbock State Supported Living Center (LbSSLC) Services may include but not be limited to: a) Inspection of interior and exterior HVAC systems and components to diagnose malfunctions b) Perform minor and/or major repairs c) Preventive maintenance d) Removal of old equipment and installation of new equipment e) Inspection and repair or replacement of duct systems f) Inspect supply and return vents Prior to repair services commencing, Contractor shall provide, for Lubbock State Supported Living Center (LbSSLC) review and approval, a detailed report and quote including the following: a) Service ticket number b) Device or equipment c) Date and time of inspection d) Technician who performed inspection: first and last name, title, and certifications e) Location and building number f) Occupancy classification g) Applicable inspection code h) Visual and function test results i) Required correction, including make, model and price of all parts
