A. The contractor shall provide a twenty-four (24) hour unit dose distribution system into which all oral legend medications (both liquid and solid form) and all intramuscular medication are dispensed in unit dose packaging. In addition, all non-legend medications dispensed as “Housestock” will be dispensed in unit dose packaging. The following medications are not required to be unit dosed: • Nitroglycerin products • Liquid tranquilizer concentrates • Reconstituted liquid antibiotics • Bulk powder If drug is not available from manufacturer in unit dose packaging, the contractor will individually package each dose, both liquid and solid medications. Each package must be hermetically sealed in compliance with FDA packaging regulations. Medication must be properly labeled with: 1. Name of drug, both trade name and generic name. 2. Strength of the drug stated in metric system. 3. Name of manufacturer and/or distributor. 4. Lot number. 5. Expiration date. 6. Total quantity delivered for Liquids (i.e. Delivers 10ml). B. All doses for each patient must be delivered in a patient tray having at least six (6) compartments. The compartments must be labeled and reflect the time periods of medication administration. The patient identification label on the front to the tray must contain the following: 1. Patient name and medical record number. 2. Attending physician name. 3. Patient location. 4. Room number. C. Contractor must supply six new medication carts, one to each unit, at no additional cost. Contractor’s cassettes containing a 24-hour supply of medication for each patient shall be delivered daily, (365 days per year), and exchanged for the empty cassettes used the previous day.
